  • 1 lb ground beef
  • 1/2 cup breadcrumbs
  • 1/4 cup grated Parmesan cheese
  • 1 small onion, finely chopped
  • 1 green bell pepper, finely chopped
  • 2 cloves garlic, minced
  • 1 egg
  • 1/4 cup ketchup
  • 1 tablespoon Worcestershire sauce
  • Salt and pepper to taste
  • 1 cup shredded provolone cheese
  • 1 cup sliced mushrooms (optional)
  • 1/2 cup sliced or diced cooked steak (optional, for extra flavor)


  1. Preheat your oven to 375°F (190°C) and grease a loaf pan.
  2. In a large mixing bowl, combine the ground beef, breadcrumbs, Parmesan cheese, onion, bell pepper, garlic, egg, ketchup, Worcestershire sauce, salt, and pepper. Mix until well combined.
